Social Work

VA Social Workers are licensed clinicians who provide information, referrals, case management, psychotherapy, and counseling services to Veterans and family members experiencing physical, emotional, and social challenges in life. At the West Palm Beach VA Medical Center, Social Workers are located in 29 different programs and areas of the health care system.
Social Work Services and Programs
BLIND REHAB PROGRAM
A comprehensive residential rehab program helping Veterans develop healthy attitudes toward themselves, blindness, and the future.
CAREGIVER SUPPORT PROGRAM
Provides a wide range of supportive services to caregivers of eligible Veterans of all eras.
COMMUNITY ADULT DAY HEALTH CARE
Provides Veterans with an opportunity for socialization and cognitive stimulation, allowing respite for the caregiver.
COMMUNITY NURSING HOME
A skilled or long-term nursing home care that can be provided at VA expense for eligible Veterans.
COMMUNITY RESIDENTIAL CARE CRC
Coordination and partnership with Assisted Living Facilities to provide placement for Veterans.
COMPENSATED WORK THERAPY (CWT) PROGRAM
Psychosocial rehabilitation program that provides realistic and meaningful vocational opportunities to Veterans recovering from chronic mental illness, chemical dependency, and homelessness.
CONTRACT RESIDENTIAL PROGRAM
Case management and residential services provided to Homeless Veterans with substance abuse and mental health diagnoses. Partnerships with Salvation Army Center of Hope and Faith* Hope* Love* Charity* Stand Down House.
GRANT AND PER DIEM (GPD) PROGRAM
Community partnership with local residential homeless agencies to provide transitional housing and supportive services to homeless Veterans.
HEALTH CARE FOR RE-ENTRY VETERANS (HCRV)
A homeless prevention service, providing education and outreach to Veterans in prisons.
HEALTH CARE FOR HOMELESS VETERANS (HCHV) PROGRAM
Outreach, referral, and placement services for at-risk for homelessness and homeless Veterans.
HOUSING URBAN DEVELOPMENT VA SUPPORTIVE HOUSING (HUDVASH)
Independent housing and case management program for homeless Veterans.
INPATIENT SERVICES
Social workers are located in the following inpatient areas: Inpatient Psychiatry, Community Living Center, Hospice and Palliative Care, and Acute Medicine. Social Workers in these areas provide discharge planning, psychosocial assessment, placement coordination, resources/referrals, end-of-life discussions and support, coordination with community agencies, and support groups.
MEDICAL FOSTER HOME (MFH) PROGRAM
Family-style living for long-term care in foster homes individualized to match Veterans' health care needs.
MENTAL HEALTH
Care and services to Veterans and families in outpatient Mental Health care settings to include Mental Health Clinic, Mental Health PACT Team, PTSD Clinic, Psychosocial Recovery and Rehabilitation Clinic PRRC, Crisis Intervention Team, Primary Care Behavioral Health, and the Substance Abuse Treatment Program.
MILITARY SEXUAL TRAUMA (MST) COORDINATOR
Education and outreach related to Military Sexual Trauma screening, support, and intervention. To access the program page click here.
TRANSITION CARE MANAGEMENT (FORMERLY OEF/OIF/OND) PROGRAM
Case management support provided to transitioning war Veterans by a dedicated team.
SPINAL CORD INJURY AND INFECTIOUS DISEASE
Provides care coordination to Veterans with spinal cord injuries/illnesses, and infectious diseases.
VETERANS JUSTICE OUTREACH PROGRAM
Coordinates services of justice-involved Veterans. Partnership with Palm Beach, Indian River, Martin, Okeechobee and St. Lucie County Courts.
